The Panel Coordinator functions as a member of the Care Coordination Team, supporting NHC providers and clinicians in managing special populations and designated panels of patients by coordinating follow up for the identified gaps in care and health maintenance needs of established NHC patients. The Panel Coordinator functions with a population health focus to help achieve positive patient care outcomes and maximize the patient’s involvement in their own plan of care.

Essential job duties:

  • Coordinates NHC patient panels by monitoring empanelment of designated panels, scrubbing proactively for health maintenance needs, identifying gaps in care, and outreaching to assist patients to re-engage with an NHC Care Team member
  • Monitors chronic disease and prevention reports and initiates follow up with patients as needed
  • Collects clinical assessment data from patients via telephone and chart review, and instructs patient treatment as directed by NHC providers
  • Acts as a liaison between patient and provider and specialists to coordinate treatment plans

Education and experience:

  • Requires completion of a Medical Assisting program and/or diploma from a Licensed Practical Nursing (LPN) program of an accredited Practical Nursing program
  • Medical Assistant Certification or unencumbered and active LPN license required
  • One-year related experience and/or training in primary care required
