The Hybrid Clinic/Telephone Triage Registered Nurse will divide their time between delivering nursing care to NHC patients in a Primary Care Clinic and via the telephone.

Essential job duties:

• Performs comprehensive health assessments of patients by completing a physical and psycho-social history. Identifies actual and potential health needs based on medical, mental/emotional, financial, and social factors
• Triages minor to emergent situations
• Respond to medical emergencies
• Performs nurse visits and administers medications and immunizations as ordered by an authorizing NHC provider or via NHC approved standing order
• Assists patient to maintain maximum quality of life by coordinating services of community agencies and encouraging preventive practices

Education and experience:

• This experience requires an Associate’s Degree in Nursing; a Bachelor’s Degree in Nursing is preferred
• This position requires completion from an accredited school of nursing
• 1-3 years related experience and/or training preferred in the FQHC or primary care field
• Bilingual proficiency in English and Spanish preferred
• 1 year of previous medical telephone triage experience preferred
• This position requires a current, unrestricted Oregon RN License
