This position is responsible for providing comprehensive clinical services and support in the outpatient management and care of patients at NHC.

Essential job duties:

• Perform comprehensive health assessment of patients by completing a physical and psycho-social history
• Order, interpret, and evaluate diagnostic tests to identify and assess patient condition
• Consult and coordinate with healthcare team members to develop and implement comprehensive patient care lan based on assessment
• Assist in evaluation of clinic procedures and protocols

Education and experience:

• Completion from an accredited licensed practical nursing program, 2+ years related experience and/or training, or equivalent combination of education and experience
• 1+ year experience in primary care preferred
• Current unrestricted Oregon LPN license
• Must be CPR/BLS certified
