Billing and Other Paperwork

Our administrative office in Portland, Oregon is open Monday through Friday from 8am to 5pm.

Release of Medical Records

To request a copy of your medical or dental records, please call the clinic business hours. Please note, a signed release form is required to process a request for a paper copy of your records. Alternatively, you can sign up for MyChart to receive immediate, electronic access to your records or email

After Hours

A nurse triage service is available after hours for patients in need of clinical advice. Call (503) 848-5861.

For prescription refills of established non-narcotic medications, please call back during business hours. Please note it may take up to 72 hours to process a medication refill. Any new medications or narcotic medications require an in-person visit with your provider before filling. If you cannot wait for a prescription refill, please visit an urgent care facility in your area.

Inclement Weather

If we are experiencing inclement weather or an adverse event, we will post NHC information about NHC clinic closures or delays on our website. If your appointment is canceled due to inclement weather or environmental event, you will receive a MyChart or phone message. You can also call (503) 848-5861 to reschedule.
