Release of Medical Records
To request a copy of your medical or dental records, please sign and return a medical or dental release form (see below) and return it to the nearest NHC clinic or by mail to our administrative office: ATTN: Compliance and Risk, 7320 SW Hunziker Rd, STE 300, Portland, OR 97223.

After Hours & Prescription Refills
A nurse triage service is available after hours for patients in need of clinical advice. Call (503) 848-5861.
For prescription refills of established non-narcotic medications, please call back during business hours. Please note it may take up to 72 hours to process a medication refill. Any new medications or narcotic medications require an in-person visit with your provider before filling. If you cannot wait for a prescription refill, please visit an urgent care facility in your area.

If we are experiencing inclement weather or an adverse event, we will post NHC information about NHC clinic closures or delays on our website. If your appointment is canceled due to inclement weather or environmental event, you will receive a MyChart or phone message. You can also call (503) 848-5861 to reschedule.
