At NHC Tanasbourne Medical & Dental Clinic, you will receive patient-centered, community-minded care no matter your ability to pay or current state of wellness. This medical and dental clinic located in Tanasbourne, Oregon offers team-based care for patients of all ages.

Services offered include primary care, family medicine, dental care, youth services, family planning, pediatrics, preventive care, chronic care management, behavioral health and mental health support, nutrition education, and medication reconciliation. We also offer trusted referrals for patients in need of specialty care.

Monday – Friday: 7:00AM – 6:00PM

Saturday and Sunday: Closed

After Hours Advice Line

A nurse triage service is available after hours for patients in need of clinical advice. Call (503) 848-5861.

For prescription refills of established non-narcotic medications, please call back during business hours. Please note it may take up to 72 hours to process a medication refill. Any new medications or narcotic medications require an in-person visit with your provider before filling. If you cannot wait for a prescription refill, please visit an urgent care facility in your area.

How to Find Us

This clinic is on the Providence Tanasbourne campus, in building C. The number 10690 can be seen at the top of the building above the large, covered entrance. Use the elevator to get to the 2nd floor. Go left out of the elevators and straight back to the end of the hall to the clinic.

Taking the Bus?

This clinic is located close to the NE Cornell & 106th Stop for the 48 Bus and the NW 185th & NE Cornell Stop for the 52 Bus.

Do you need transportation assistance?

If transportation is an issue, healthcare transportation assistance may be available. Call the clinic or talk to a receptionist for help. Some insurance plans cover the cost of transportation to and from the clinic. Bus passes, gas cards, or other reimbursements for travel to and from the clinics, may also be available.

Rebekah Bartholomew, PharmD

Experience:  Completed residency in the Advanced Education in General Dentistry Postdoctoral Program providing comprehensive care in a community clinic setting, in 2019 completed a service trip to Guatemala to provide dental care to underserved populations in Antigua and surrounding communities
