At NHC Merlo Station HS SBHC, you will receive patient-centered, community-minded care no matter your ability to pay or current state of wellness.  NHC Merlo Station HS SBHC offers team-based care for Beaverton School District students. We also offer trusted referrals for patients in need of specialty care.

Hours of Operation

Monday – Wednesday:
8:00AM – 4:30PM
(closed 12:00PM – 1:00PM for lunch)

Thursday through Sunday: Closed

On-Call Provider: If the clinic is closed and you are a clinic patient in need of clinical advice, please dial (503) 941-3210 and request to speak with the on-call provider.

Our Services

  • Diagnosis and treatment of minor illness, injuries, colds, ear infections, rashes, asthma, and sinus infections (may include limited labs, prescriptions, and handouts/information)
  • Wellness/health physical exams or check-ups
  • Sports physicals
  • Immunizations
  • Blood pressure or vision screenings
  • Mental health evaluations and counseling
  • Health counseling (may include nutrition, tobacco use/cessation, weight management, safety, wellness, substance and/or drug dependence, and health risk assessment/identification/reduction
  • Addressing safe and responsible sexuality, menstrual disorders, and testing for pregnancy

No insurance? No problem. We offer discounts on all our services and all patients are encouraged to apply.

Celeste Craven, FNP

Education:  M.S.N.  Chamberlain University, Addison, IL

Experience:  Over 15 years of experience in a diverse array of healthcare environments, including trauma, emergency, general pediatric inpatient, and case management for women and children’s services, including as an RN in the Pediatric Emergency Department at Cardon Children’s Medical Center in Mesa, AZ

At NHC:  Provide primary care services for patients of all ages, backgrounds, and current states of wellness

My Clinics:  Merlo Station SBHC and Tanasbourne

What I do for Fun:  Spend time with family and go hiking and camping

Holly Fay, DNP, APRN, FNP-C

Education:  DNP, Gonzaga University, Spokane, WA

Experience:  Family Nurse Practitioner at the Native American Rehabilitation Association providing culturally competent primary care to Native Americans, Native Alaskans, and other vulnerable populations and over 12 years of experience as an RN working in a hospital setting

At NHC:  I provide educational support and health care to patients of all ages as a Family Nurse Practitioner

My Clinics:  Beaverton and Merlo Station

What I do for Fun:  Take weekend trips to the beach, go camping, cook, and spend time with family
