At NHC Beaverton Medical Clinic, you will receive patient-centered, community-minded care no matter your ability to pay or current state of wellness. Services offered include primary care, family medicine, preventative care, chronic care management, behavioral health and mental health support, nutrition education, and medication reconciliation. We offer trusted referrals for patients in need of specialty care.

Hours of Operation

Monday – Friday: 7:00AM – 6:00PM

Saturday and Sunday: Closed

After Hours Advice Line

A nurse triage service is available after hours for patients in need of clinical advice. Call (503) 213-3800.

For prescription refills of established non-narcotic medications, please call back during business hours. Please note it may take up to 72 hours to process a medication refill. Narcotic medications also require an in-person visit with your provider. If you cannot wait for a prescription refill, please visit an urgent care facility in your area.

How to Find Us

This clinic is in the Suncor Plaza next to the Beaverton Walmart Neighborhood Market. The clinic is immediately inside the front entrance to the right. The front window sign still says, “Beaverton Family Medicine Ste 110."

Taking the Bus?

This clinic is easy to access via public transportation. The NW Cornell & 173rd Ave Stop on the 48 Bus is directly in front of the clinic.

Transportation Assistance

If transportation is an issue, healthcare transportation assistance may be available. Call the clinic or talk to a receptionist for help. Some insurance plans cover the cost of transportation to and from the clinic. Bus passes, gas cards, or other reimbursements for travel to and from the clinics, may also be available.

Holly Fay, DNP, APRN, FNP-C

Education:  D.N.P.  Gonzaga University, Spokane, WA

Experience:  Family Nurse Practitioner at the Native American Rehabilitation Association providing culturally competent primary care to Native Americans, Native Alaskans, and other vulnerable populations and over 12 years of experience as an RN working in a hospital setting

At NHC:  I provide educational support and health care to patients of all ages as a Family Nurse Practitioner

What I do for Fun:  Take weekend trips to the beach, go camping, cook, and spend time with family

Karla Goodman, ND

Education:  N.D.  Southwest College of Naturopathic Medicine, Tempe, AZ

Experience:  Practice naturopathic medicine in assisted living pathways and family and elementary school clinics, addressing conditions and illness through the holistic care of mind, body, and spirit

At NHC:  I provide whole-person naturopathic medicine in family practice, pediatrics, women’s medicine, and endocrinology

Amanda Kruszka, LCSW

Education:  M.S.W.  Grand Valley State University, Allendale, MI

Experience:  10 years of experience supporting emotional and mental health needs of individuals of all ages, including those coping with chronic conditions, domestic violence, sexual assault, and struggling with various life or situational stressors

At NHC:  I provide Behavioral Health Services to diverse patients, which includes recommendations to community resources and short-term counseling, education, guidance, and support to assist patients with their health and wellness goals.

Sakura Porter, PharmD, BCACP

Education:  PharmD.  Oregon State University, Corvallis, OR | PharmD.  Oregon Health & Science University, Portland, OR | Board-Certified Ambulatory Care Pharmacist

Experience:  More than six years of experience counseling and advising patients, developing treatment protocols, and working with patients for medication reviews, patient education, and disease state management

My Clinics:  Beaverton and Canby

What I do for Fun:  Foster rescue dogs – over 25 from the Navajo reservation where I last worked (I adopted the first two dogs I fostered named Kei-chan and Yumi!)

Tyron Tracy, PA-C

Education:  M.M.S.  Arcadia University, Christiana, DE

Experience:  Two years in the Peace Corps, as well as time spent as an endocrinologist, volunteer EMT, and physician’s assistant in urgent care and family medicine

At NHC:  I care for patients ages three days old to 95 years old as a family medicine physician’s assistant with a focus on educating patients on health issues and ways to continue improving health outcomes

As of Monday, February 7, 2023, NHC Beaverton Medical Clinic will officially merge with NHC Tanasbourne Medical and Dental Clinic.

We are proud to announce this merger as it prioritizes the care you receive and you’ll have access to additional services at this larger clinic.  All assigned Beaverton Medical Clinic patients will automatically become patients of our Tanasbourne Medical Clinic — no additional information or action needed on your part.

We have answered a few of the most common questions below, and we welcome any additional questions or concerns you may have.  Please contact your care team or call (503) 848-5861 with questions or concerns.

FAQs

Will I get a new care team?

Your current care team will still be available when we move to our Tanasbourne location, but you will have access to a more diverse population of providers and specialties.

Will my scheduled appointment be affected?

If you have an in-person visit scheduled at the Beaverton Medical Clinic between now and February 3, 2023, your appointment will take place as usual and will not be affected by this plan.  In-person visits scheduled for on or after February 7, 2023 will be located at the Tanasbourne location.

Will this affect the cost of services?

This merger will not affect the cost of services or require additional paperwork on your part.  We are working hard to provide a seamless transition of care.
