Your NHC care team will be trusted guides and partners along your personal journey to wellness. Find what to expect, forms, and how start your 24/7 online patient portal ahead of time. Please review the information below, so you are ready for your first clinic visit.

Remember to bring your completed new patient forms to your first appointment.

What to Bring

  • Completed patient registration forms. Please download and fill out these forms before you arrive for your first appointment at your Oregon NHC clinic.
  • Insurance card* and a Photo ID
  • List of current medications, including prescriptions, over-the-counter drugs, herbal supplements, or vitamins. You may also want to bring your prescription bottles.
  • List of previous healthcare providers and contact information 
  • Your top health concerns, so you can discuss these with your NHC provider

*If you do not have insurance or would like to apply for a discount, please read more about our financial assistance program. If you think you may qualify for assistance, please also bring these items to your first appointment:

  • Your last two (2) paycheck stubs OR previous year's tax return
  • Unemployment statement

Arrive Early

Try to arrive 30 minutes before your appointment time. This will give you time to complete any additional paperwork.

If you think you may be late for your appointment, please call to let us know. We will do everything possible to accommodate you.

If you are more than 15 minutes late to your appointment, we may need to reschedule your visit.

Uninsured Patients

Our trained Enrollment Assisters are here to help you understand your options, and apply for a health insurance plan that works best for you. This help is free. Contact us and ask for Enrollment Assistance.

Sliding Fee Discount

We offer a sliding fee discount for eligible patients based on family size and income. Please bring a $25 down payment to your first visit. Learn more about NHC financial assistance options.

Insurance Accepted

We accept CareOregon, Medicaid, and other insurance carriers like Delta Dental. If a co-pay is due, we will collect it at your visit. We bill your insurance directly, and you will be responsible for the balance.

Translation Services

Our teams speak English and Spanish. Other translators are available to patients at no cost. Please call (503) 848-5861 to request translation services for your first visit.

Help with Chronic Pain

NHC providers never prescribe opioid or narcotic medication on the first visit. We use evidence-based practices and follow community standards in the treatment of chronic pain. View our Opioid Medications Policy and Guidelines.

Good Faith Estimates

Beginning January 1, 2022, all healthcare providers must provide something called a Good Faith Estimate. This estimate will explain how much care will cost for patients, who are uninsured or who wish to self-pay.
