Dan holds a Master of Science in Rehabilitation Counseling as well as a Bachelor of Science in Psychology from Portland State University in Portland, Oregon, and is a US Army Veteran. Dan has over 10 years of experience serving diverse populations including military veterans, victims of trauma, individuals with development disabilities and mental illness. His career has included roles as a vocational rehabilitation counselor, residential case manager and a clinical specialist for a national suicide prevention hotline. Before moving to Oregon to attend college, he was a disc jockey and special education teacher. Dan provides Behavioral Health Services in our Milwaukie Clinic..
Gregory earned a Doctor of Medicine from Jefferson Medical College of Thomas Jefferson University in Philadelphia, Pennsylvania in 2010 and completed his Family Medicine Residence at Oregon Health and Sciences University (OHSU) in Portland, Oregon in 2013. Gregory also holds a BA in Economics, with a minor in Chemistry from Brown University in Providence, Rhode Island. Previously, Gregory was the Chief Resident at the OHSU Family Medicine Residency Program and is a Clinical Instructor for OHSU. He is a “Reach Out and Read” Certified Instructor and volunteers his time to train providers to utilize the program’s format to promote early childhood reading. Greg provides patient care in our Milwaukie Clinic.
Claire holds a Master of Science in Nursing from Yale University School of Nursing, New Haven, Connecticut; and a Bachelor of Arts in Psychology from Smith College, Northampton, Massachusetts. Claire has over 5 years of experience in the healthcare field, including 1.5 years in community health. She has clinical experience with pediatric anesthesia, pediatric orthopedic surgery, geriatric care services, primary care services for correctional institute inmates, in-home patient care services and camp nurse for children 8-15. Claire has traveled internationally pursuing her interest in healthcare, including working in Nicaragua, Uganda, and South Africa. Claire’s personal interests include running, cooking and traveling.
Uche holds a Doctor of Pharmacy from the University of Colorado, Skaggs School of Pharmacy and Pharmaceutical Sciences in Denver, Colorado, and a Bachelor of Pharmacy from the University of Benin, School of Pharmacy in Benin City, Edo State, Nigeria. Uche has over 10 years of experience providing culturally sensitive pharmaceutical care to underserved and high risk patients. She is currently involved in standardizing medication reconciliation processes, providing recommendations for optimal use of medications to clinic providers and collaborative drug therapy management of disease conditions like diabetes, hypertension, dyslipidemia and tobacco use disorder. In her spare time she enjoys running and hiking with her family. She provides clinical pharmacy services in our Milwaukie and Canby Clinics.
Sarah holds a Doctorate in Medicine from Keck School of Medicine of the University of Southern California, and received a BA in Comparative Literature from the University of California at Berkeley. Sarah did her residency at Oregon Health and Science University in Family Medicine and has special interests in geriatrics, women’s health, physician wellness, and chronic disease management. In her personal time Sarah enjoys skiing, camping, hiking, yoga, dancing, literature and travel. She joined Neighborhood Health Center in 2013 and provides services in our Milwaukie Clinic.
