Neighborhood Health Center has added Clinical Pharmacy Services to its clinics. This service is staffed by highly trained Clinical Pharmacists who work with you and your care team to help you get and stay healthy. These pharmacists are specialists in medical drug therapy and promote high quality patient care through cost effective medication use.
Annual Medication Reviews
Patients who take multiple medications to treat chronic health conditions may be referred by their Provider to have a yearly visit with a clinical pharmacist for an Annual Medication Review. At this visit, the pharmacist will review and update your medication history in your health record, evaluate how your medications are working for you, screen for drug interactions and side effects, and answer any questions you might have about your medications. Our pharmacists look for ways to make it easier for you to take your medications and improve your health at the lowest cost to you.
Collaborative Drug Therapy Management
Working with your Provider, the clinical pharmacists may also provide medication management of chronic health conditions such as diabetes, hypertension, high cholesterol and tobacco use. More frequent visits with the pharmacist in between your Provider visits, we will be able to more closely manage your illnesses and provide education on self-management and healthy behaviors.
Your Provider may also refer you for a pharmacy consult if you have questions or concerns about your medications, or have difficulty obtaining or using the medications prescribed to you.
Our pharmacists can help you gain access to specialty or high-cost medications, understand what prescriptions your insurance will cover and recommend alternate medications when access is limited by cost, supply or insurance.
